Supporting Kindergarten Readiness Through Celebration & Literacy
As children approach their fifth birthday and complete their final Reach Out and Read well-visit, the Graduation Toolkit serves as a meaningful milestone marker, celebrating the end of their participation in the program and the beginning of their journey into kindergarten. Designed for pediatric providers, this toolkit offers engaging ways to acknowledge this transition while equipping families with fun, practical resources to continue nurturing their child’s love of reading at home.
Grounded in early literacy and developmental best practices, the toolkit helps clinics recognize this key developmental moment, strengthen caregiver relationships, and send families off with encouragement and tools for the exciting path ahead.

What Is the ROR Graduation Toolkit?
The Graduation Toolkit is a resource package developed by Reach Out and Read Illinois to help providers celebrate young readers while guiding families through the school readiness journey.
Clinics can use these materials during well-child visits, graduation moments, and waiting room engagement to highlight the importance of reading and developmental growth.
What’s Inside the Toolkit?

The Children’s Activity Book
A colorful, engaging workbook filled with games, puzzles, and interactive challenges that build school-readiness skills such as:
- Fine motor control
- Counting and number recognition
- Letter awareness
- Emotional confidence
- Creativity and imagination
Children also receive a graduation certificate to celebrate their achievements.


The Parent Resource Booklet
A practical, easy-to-read guide for parents, guardians, and caregivers covering:
- Signs of kindergarten readiness
- How to choose the right kindergarten program
- Daily routines for a smoother school transition
- Emotional support and social-emotional development
- Tips to keep the love of reading alive at home

An Adventure Map Poster
A fun illustrated poster featuring Rory the Lion exploring everyday reading spots like the park, library, and doctor’s office. Ideal for clinic waiting rooms to encourage literacy everywhere.

A Selfie Photo Frame
A large, cheerful frame for clinics to use in celebrating each child’s graduation moment. Capture and share the joy as children prepare for kindergarten!
How to Use the Toolkit
Providers can:
- Hand out activity books and parent guides at 5-year well-child visits
- Display the poster and frame in the clinic to create a festive atmosphere
